Please bring Form I-693 to your exam. The government requires this for the completion of your immigration physical.

This immigration exam can be conducted only by USCIS appointed doctors. Suncoast Medical Group additionally offers vaccination assessments and related services to refugees.
You are requested to bring the I-693 form when coming to the exam. This is needed by the government to complete the immigration physical.

If there is a need for further testing and vaccinations for certain medical illnesses recognized during the course of the medical exam, Suncoast Medical Group offers necessary provisions for blood testing, vaccinations, and x-ray facilities at the same visit, depending upon the availability of the applicant**.
